⏱ About 5 minutes
SKIP THIS PAGE IFYou made no tax payments of your own toward 2026 and no part of your 2025 refund was carried forward. Tick the box at the top and move on.
YOU NEED THIS PAGE IFYou sent the IRS or a state a quarterly payment, paid something with an extension request, or applied last year’s refund to this year.
WHAT THIS PAGE DOESRecords every payment you already made toward your 2026 tax, to the IRS and up to two states. If a payment is missed here, your return will show a balance due you’ve already paid.
These payments are credits against what you owe, so every one you list here lowers your balance due.
1 · Before you begin
Gather these first. Tick them off as you go.
IMPORTANTDon’t list tax withholding from a W-2, pension or Social Security here. This page is only for payments you made yourself. Withholding is reported elsewhere in the organizer.

3 · Common mistakes that cause delays
The issues we see most often on this page.
Counting a refund applied forward twiceEnter it only in the Refund Applied Forward box, not again as a quarterly payment.
Missing the 4th-quarter paymentIt applies to 2026 but is usually paid in January 2027.
Using the wrong payment dateUse the exact date from your bank statement or confirmation. The wrong date can trigger penalties that don’t apply.
Entering withholdingW-2, pension and Social Security withholding doesn’t belong on this page.
Leaving out a third stateThe page has room for two states. Put any others in the comments.
4 · When you’re finished
- Compare the amounts on Page 4 with your bank records so every payment is included.
- Not sure a payment belongs here? Add a note in the comments and we’ll review it.
